We are looking for two RN Case Managers to join our onsite team in San Leandro, CA!
Unit: Social Services
Schedule: Monday - Friday w/ weekend rotation, 8am - 430pm
Contract Term: 13 weeks to start
Job Description: Clinical case management w/ heaving focus on discharge planning in an acute care hospital setting
Requirements:
2 years acute hospital experience is CM & Discharge Planning
Willing to work weekend rotation
Inpatient case management experience
BLS - American Heart Association
EPIC
Allscripts/Careport
Interqual
Utilization Review experience is a plus
ACM or CCM Certification is a plus
