JOB SPECIFICS
We currently have one Registered Nurse Social Services position in the Access, Aging, and Disability (AA&D) Division of Community Services on the Intake and Assessment team. This position will be providing initial MnCHOICES Assessment services to people with varying abilities and ages. Services are provided to a broad population using an inclusive, person-centered approach that includes race equity. Qualified candidates with diverse perspectives are encouraged to apply. One or more years of experience in Home and Community-Based Services (HCBS) is preferred for MnCHOICES Certification. Proficiency with technology, strong organizational skills, and passion for supporting people in our community are essential for this fast-paced, rewarding position. This position requires a Bachelor's degree in nursing with current licensure as a registered nurse along with public health certification and at least one year of home and community-based experience, OR current license as a registered nurse with at least two years of home and community-based experience. DESIRABLE EXPERIENCE:
Experience interacting with and providing customer service to individuals from a variety of cultural and ethnic backgrounds
Certified MnCHOICES Assessor with direct assessment experience
One or more years of experience in Home and Community Based Services (HCBS-waivers) is required for MnCHOICES certification; Direct experience with HCBS and Minnesota Health Care Programs
Four-year degree in nursing with public health nursing credential
Work experience in a county agency providing public health nursing
Demonstrated experience in understanding, implementation, and analysis of person-centered practice and diversity, equity, and inclusion work
Experience and comfort facilitating and/or participating in group work
Ability to efficiently use time and technology to complete all assessment components within required timelines, while applying person centered best practices and ensuring the person understands all resources and options available to them
Work experience providing nursing services in a related agency, such as a home health agency, nursing facility, or assisted living
HOURS/DAY S: 8:00 a. m.-4:30 p. m., Monday-Friday based out of the Stillwater Office. Work schedules (hours and days) and location are subject to change based on department needs. The position offers teleworking and flexible hours that align with the business need.

ANTICIPTATED HIRING RATE : $70,054.40 - $95,576.00 Annually

JOB SUMMARY
The Registered Nurse Social Services determines client eligibility, provides skilled nursing for assessments, and manages cases and care coordination for the waiver programs by implementing plans to protect vulnerable people and promote independence.
EMPLOYMENT CRITERIA
MINIMUM QUALIFICATIONS: This position requires a two-year degree in nursing from an accredited nursing school, AND a Minnesota Registered Nurse License.
