About the Opportunity
Discovery At Home is seeking a PRN Speech-Language Pathologist (SLP) to support patients in the Bradenton area. This role provides skilled speech-language pathology services in patients' homes in accordance with physician orders and established plans of care.
You will work collaboratively with an interdisciplinary care team to assess, treat, and educate patients with communication, cognitive-linguistic, voice, and swallowing disorders to improve safety, independence, and quality of life.
Key Responsibilities- Perform initial and ongoing comprehensive speech-language pathology evaluations, including OASIS assessments as required
- Develop, implement, and update individualized speech therapy plans of care in collaboration with physicians and the interdisciplinary team
- Establish measurable goals based on patient needs, functional deficits, and rehabilitation potential
- Provide skilled treatment for speech, language, cognitive-communication, voice, and swallowing disorders
- Perform dysphagia assessments and implement safe swallowing strategies in accordance with physician orders
- Educate patients and caregivers on communication strategies, cognitive techniques, swallowing precautions, and home programs
- Assess home environments and provide recommendations to promote patient safety and carryover of therapy goals
- Communicate regularly with physicians regarding patient progress and obtain updated orders as needed
- Complete visit notes, progress notes, reassessments, and discharge documentation within agency-defined timeframes
- Participate in discharge planning and complete Speech Therapy discharge summaries
- Attend patient care conferences, in-services, and Quality Assessment and Performance Improvement (QAPI) activities as assigned
- Maintain compliance with all agency policies, state regulations, and Medicare guidelines
Assists in Agency Admission Process:
- Obtains a medical history, particularly as it relates to the present condition.
- Evaluates the client, and the home situation to determine what instruction and assistance will be available from the family in caring for the client, and what other Agency and community services will be required.
- Assures that the physician's orders are appropriate and discusses necessary changes.
Qualifications:
- Master's degree in Speech-Language Pathology from an accredited program
- Current Certificate of Clinical Competence (CCC-SLP) from ASHA
- Current Speech-Language Pathologist license in the state of practice
- Minimum 1 year of clinical experience
- Home health experience preferred
- Experience with adult and geriatric populations preferred
- Strong clinical judgment, documentation, and communication skills
- Ability to work independently and manage PRN scheduling effectively
