About the Opportunity

We are seeking a PRN Occupational Therapist to join our growing team in the Bradenton area. In this role you will provide skilled occupational therapy services on an intermittent basis to clients in their homes in accordance with the physician's orders. 

You'll work collaboratively with an interdisciplinary team to promote safety, independence, and improved quality of life for patients in the home setting.

Responsibilities:

  • Perform initial and ongoing comprehensive occupational therapy evaluations, including OASIS assessments as required
  • Develop, implement, and update individualized occupational therapy care plans in collaboration with physicians and the interdisciplinary team
  • Establish measurable goals based on patient functional status and potential for improvement
  • Provide skilled interventions to improve ADLs, mobility, safety, and independence
  • Educate patients and caregivers on therapeutic techniques, adaptive equipment, and safety strategies
  • Instruct patients in the use of assistive devices, splints, braces, and orthotics
  • Assess home environments and recommend modifications to support patient safety and function
  • Supervise Occupational Therapy Assistants (OTA) at least monthly and Home Health Aides bi-weekly when OT is the sole discipline involved
  • Communicate regularly with physicians regarding patient progress and obtain updated orders as needed
  • Complete visit notes and clinical documentation within agency-defined timeframes
  • Participate in discharge planning and complete Occupational Therapy discharge summaries
  • Attend care conferences, in-services, and QAPI activities as assigned
  • Maintain compliance with all agency policies, state regulations, and Medicare guidelines

Qualifications:

  • Graduate of an Occupational Therapy program approved by the American Medical Association (AMA) in collaboration with the American Occupational Therapy Association (AOTA)
  • Current Occupational Therapist license in the state of practice
  • Minimum 1 year of institutional experience
  • Minimum 2 years of home health experience (preferred)
  • At least 1 year of supervisory experience (preferred)
  • Strong clinical, organizational, and communication skills
  • Ability to work independently and manage PRN scheduling effectively
