Home Health Nurse Coordinator, Full-Time to work under the direction of the Manager of Home Health and within established organizational and departmental policies and procedures, responsible for providing skilled care and case management to clients/families in accordance with the State Nurse Practice Act and accepted professional nursing standards of practice. #The Home Health Nurse Coordinator provides services using best practice guidelines, communicates/documents observations and assessments, promotes and maintains quality of life for clients/caregivers through direct care, teaching, and counseling following the interdisciplinary group plan of care. The Home Health Nurse Coordinator assists with the orientation and professional development of team members and provides clinical oversight to the on-call and weekend nurses, in conjunction with the Home Health Manager. MINIMUM REQUIREMENTS: Graduate of an approved, accredited school of professional nursing, BSN preferred.# Current licensure to practice as a Registered Nurse in the Commonwealth of Massachusetts is required.# Current CPR certification required. Minimum of one (1) year of medical/surgical nursing and hospice/home care nursing experience required; supervisory, educational, and/or quality department experience preferred. Demonstrated excellent verbal and written communication skills required.# Demonstrated strong interpersonal, organizational and time management skills required. Demonstrated knowledge and understanding of the dying process with an ability to identify and treat signs and symptoms of the same, including competency in pain and symptom management required. A current driver#s license, good driving record and reliable transportation are required.
