This position is responsible for program development and implementation within the Memory Care Program.  Responsible for providing physical and emotional support to each Memory Care resident while maintaining a safe and comfortable home like environment.

Responsibilities:

  • Designs, schedules and facilitates Memory Care program incorporating Life Skills and a variety of dementia appropriate activities.
  • Partners with the RDRC to ensure a variety of appropriate activities are available throughout the day and evening and that Care Managers are actively involved and engaged with activities.
  • Partners with the RDRC in compiling, coordinating and executing a comprehensive and varied program of activities to meet the broad spectrum of interests and capabilities of Memory Care residents
  • Provide ongoing in-service education to the Memory Care staff that is relative to the disease processes and population being served
  • Prior to move-in, reviews the resident's preferences and needs educating Care Managers of same.
  • Facilitates the Service Plan and Daily Assignment Sheet development to ensure excellence in service delivery, safety, hydration and resident engagement in programming.
  • Acts as the community champion in Memory Care programming and activities.
  • Partners with dining services to ensure meeting individualized resident needs and preferences; participates in dining experience.
  • Understands resident changes in condition and reports to the nurse.
  • Establishes a cooperative relationship with the local Alzheimer's Association chapter or other Alzheimer's advisory organization.
  • Develops specific programming and approach mechanisms that are tailored to meet the needs of residents with a diagnosis of Alzheimer's Disease or related Dementia.
  • Creates a therapeutic environment focusing on first impressions and resident engagement

Quality Assurance and Regulatory Compliance

  • Strives for excellent quality care and service delivery.
  • Develops and implements appropriate plans of action to correct identified deficiencies and other regulatory compliance.
  • Develops a thorough working knowledge of current and evolving state laws, regulations, policies and procedures dictated for residents and ensures compliance.

Care Giver:

  • Responsible for a designated group of residents during the shift; knows where their residents are and physically checks on them throughout the shift.
  • Observes, reports and documents symptoms and conditions of residents for changes in condition such as skin, behavior, alertness, weight, dietary and participation in activities.
  • Responds to security system and resident call bells promptly.
  • Notifies supervisor and/or Health Care Coordinator if a resident has increased care needs.
  • Assists with continence management and disposes of all continence products properly to ensure sanitation of resident suite and community restrooms.
  • Serves and collects food trays and assists as needed; reports refusal of meals or loss of appetite.
  • Maintains cleanliness of resident's room and work areas.
  • Practices good standard care precautions of cleanliness, hygiene and health.
  • Helps residents maintain independence, promotes dignity and physical safety of each resident.
  • Actively participates/leads and assists residents with activities of daily living (i.e. bathing, dressing, toileting, grooming, ambulation, transferring, eating) as instructed.
  • Engages residents in life skills and other life enrichment activities.
  • Greets and assists all internal and external customers, guests, family members, residents, vendors and team members.
  • Does resident laundry as assigned and needed.

Qualifications:

  • One (1) year dementia care experience.
  • One (1) year in a leadership role.
  • Previous caregiver experience
