PURPOSE OF YOUR JOB DESCRIPTION: The primary purpose of your position is to direct nursing care to the residents, and to supervise the day to day nursing activities performed by Certified Nursing Assistants. Such supervision must be in accordance with current federal, state and local standards, guidelines and regulations that govern our facility, and as may be required by the Director of Nursing, Nursing Supervisor or Clinical Coordinator to ensure that the highest degree of quality care is maintained at all times. DELEGATION OF AUTHORITY: Under the direction of the Director or Nursing, Nursing Supervisor, and Clinical Coordinator you are delegated the clinical authority, responsibility and accountability necessary for carrying out your assigned duties. JOB FUNCTIONS: Every effort has been made to make your job description as complete as possible. However, it in no way states or implies that these are the only duties you will be required to perform. Functions within the limits of the State Nurse Practice Act and WNR administrative and nursing department policies and procedures. Cooperate with other resident services when coordinating nursing services to ensure that the residents' total regimen of care is maintained. Initiates plan based on nursing assessment & applies knowledge of specific illnesses, injuries, diseases, human behavior and appropriate standards of care within 24 hours of admission. Involved in developing a discharge plan that utilizes all available resources. Transcribes and implements physician orders written, verbal, and electronic. Documents nursing intervention, resident response, effectiveness, complications, act and communicates this information to peers and physician. Performs treatments as ordered and documents appropriately in EMR &/or treatment book. Record new/changed diet orders. Has knowledge and understanding of MMQ documentation and completes required documentation/summaries in an accurate and timely manner. Participates in the MDS as directed by the MDS coordinator Completes required assessments in a timely manner including admissions, discharges, Medicare assessments and any follow-up notes. Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the residents, as well as the residents' response to care. Utilizes only acceptable abbreviations as noted under Nursing Policy. Complete chart check as required, making sure all physician orders have been noted accurately. Maintain the daily census report. Prepare and administer medications as ordered by the physician. Verify the identity of the resident before administering medication/treatment. Review medication cards for completeness of information, accuracy of the transcription of the physician's order, and adherence to stop order policies. Dispose of drugs as required, and in accordance with established procedures. Order prescribed medications and supplies in accordance with established policies. Demonstrates ability to initiate IV therapy as prescribed. Completes control drug count according to established procedure at change of shift and/or when leaving the building during your shift. Administer professional services such as catherization, tube feedings, suction, applying and changing dressings, colostomy care etc. Give and receive nursing report at the beginning and end of your shift, and share pertinent information with CNA staff as necessary. Ensure that all nursing care is provided in privacy. Knock before entering a resident's room. Demonstrates organizational skills in providing nursing care and demonstrates the ability to adapt to changing workloads. Daily assignments are completed within an eight-hour shift without compromising the quality of care. Provides comfort, care and dignity to the dying resident. Reports all accidents/incidents that occur in assigned area. Use restraints only when there is a completed restraint assessment, a physician's order and consent. Check restrained resident every 30 minutes and release restraint every two hours. Keeps clinical coordinator and/or supervisor informed of the resident's status. Develop work assignments for CNA staff and assigns tasks as required. May serve as a team leader for CNA and provides leadership to nursing personnel assigned to your unit/shift. Notify nursing personnel of new admissions, discharges or LOA's. Review complaints/concerns made by the nursing personnel assigned to your unit/shift and intervene or make appropriate report to supervisor as necessary. Notify the resident's primary physician and family when resident is involved in an accident/incident and record notification on event and/or in the nurse note. Review care plans to determine if changes in the resident's daily care routine have been made on the care plan, update as necessary. Encourage physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established procedures. Respects the dignity of resident, acting as an advocate. Provides a safe and secure environment for the resident. Serve food trays. Assist with feeing as indicated (i.e. cutting foods feeding, assist residents with sight problems to identify food arrangements etc,), records food intake. Make sure that departmental personnel, residents and visitors follow established policy and procedures. Interpret facility policies and procedures for personnel, residents, visitors and government agencies as required. Use only equipment that you have been trained to use, and operate this equipment in a safe manner. Reports defective equipment. Maintains compliance with OBRA regulations. Has been educated in and adhere to standard precautions; demonstrates good hand washing procedures, and follows established isolation techniques when required. Follow established safety procedures in the performance of all duties and report all safety violations to the safety officer. Attend and participate in scheduled training, educational and orientation programs. Assists in orientation of newly employed nursing staff as directed. Participates, as assigned, in the external and internal disaster plan. Maintains CONFIDENTIALITY of all pertinent resident care information to assure that resident's rights are protected. Assists QA and QI Committee in developing and implementing appropriate plans to correct identified deficiencies. Communicates effectively with coworkers, residents, families and visitors. Projects a positive attitude and respect toward staff and the facility.
