General Summary of Position
The CenteringPregnancy Program Coordinator organizes functions and activities related to patient access and service delivery as well as care management among the CenteringPregnancy Program. The Program Lead works with stakeholders in the Women's and Infants' Services Department to recruit patient participants, organizes and schedules sessions, and monitors and collects data to ensure service delivery objectives are met and outcome targets achieved. Will oversee documentation of program services delivered and participate in activities to evaluate program outcomes and prepare grant reports. Provides support for team members to effectively execute program services.

Primary Duties and Responsibilities

 

  • Preterm Birth Reduction Initiative: Creates and maintains CenteringPregnancy documentation of adherence to project protocols for service delivery and metrics to evaluate success. Develops and maintains department reports for tracking.
  • Preterm Birth Reduction Initiative: Monitors CenteringPregnancy program delivery and interventions to ensure compliance and quality. Provides regular reports to WIS Research Program Manager to jointly identify areas for improvement and determine operational adjustments as needed to ensure success of interventions.
  • Preterm Birth Reduction Initiative: Serves as a point of contact for internal and community partners to ensure seamless integration of referrals and transitions in care. Contributes to collaborative initiatives undertaken with partner organizations. When assigned may represent the CenteringPregnancy Team and/or the Preterm Birth Reduction Initiative team at internal and community meetings.
  • Preterm Birth Reduction Initiative: Organizes CenteringPregnancy schedule participates in Preterm Birth Reduction Initiative management meetings community partner linkages and program planning meetings. Take minutes and facilitates follow-up.
  • Preterm Birth Reduction Initiative: Conducts CenteringPregnancy sessions in collaboration with Provider Facilitator.
  • CLINIC OPERATIONS: Collaborates with Perinatal Program Coordinator and clinic personnel on administrative matters relating to personnel operations for the CenteringPregnancy program including all aspects of patient scheduling registration and management of office patient work-flow.
  • CLINIC OPERATIONS: In collaboration with perinatal care coordinators performs regular quality assurance activities to ensure services are delivered as intended and patients receive optimal care.
  • CLINIC OPERATIONS: Organizes and maintains program calendar of patient activities. Promotes patient awareness and utilization via clinic communication systems.
  • CLINIC OPERATIONS: Assists with responding to patient concerns and facilitates resolution of service breakdowns. Monitors patient satisfaction results reviews with staff and collaborates in process improvements.
  • CLINIC OPERATIONS: Assists with coordinating personnel activities including interviews orientation and training professional development activities and quality assurance initiatives.
  • CLINIC OPERATIONS: Monitors CenteringPregnancy facility needs to ensure a clean safe environment for team members.
  • IT SYSTEM ADMINISTRATION: Maintains current knowledge of technical developments with IT program supports including MedConnect and communicates relevant information to the group. May provide internal training and support for these systems.
  • IT SYSTEM ADMINISTRATION: Coordinates patient services alignment in MedConnect systems and facilitates continuity of care objectives. Collaborates with front line registration and provider support teams to promote CenteringPregnancy.
  • Serves as program liaison with central registration and MedConnect implementation teams. Provides team training as needed to ensure optimal systems utilization accurate documentation and appropriate follow-up.

Minimal Qualifications
Education

  • Associate's degree required
  • Bachelor's degree preferred
  • Six or more years of relevant experience may be used in lieu of the degree requirement.

Experience

  • 3-4 years of progressively more responsible job-related experience in the oversight of front desk or back-end clinic operations and/or billing and/or scheduling required
  • Prior experience with MedConnect (Cerner) and IDX required

This position has a hiring range of

USD $59,820.00 - USD $101,836.00 /Yr.
