General Summary of Position
The Director of Pharmacy for Home Infusion is responsible for the overall operational clinical and regulatory oversight of all pharmacy services provided to home infusion patients. This includes ensuring compliance with all federal state and local laws USP General Chapters <797> and <800> standards and ACHC accreditation requirements. The Director leads a team of pharmacists and pharmacy technicians to deliver safe effective and high-quality sterile compounded preparations and comprehensive pharmaceutical care to patients in their homes. Establishes policies procedures standards and objectives for the provision of services and coordinates with other agencies to provide the maximum level of pharmacy services to patients caregivers and home care staff. Markets services to internal and external clients. Assists in the medication management program at MHHI and contacts physicians and patients to review medications and perform medication reconciliation

Primary Duties and Responsibilities

 

  • Implements and maintains pharmacy programs in accordance with current practice standards and strict adherence to regulatory and standard-setting and accrediting bodies. Ensures continuous compliance with all relevant federal state and local pharmacy laws and regulations (e.g. Board of Pharmacy DEA). Maintains strict adherence to USP General Chapter <797> for sterile compounding and USP General Chapter <800> for hazardous drug handling. Serves as the primary leader and contact for ACHC accreditation readiness maintaining compliance with all ACHC standards and preparing for and participating in surveys. Oversees the maintenance of all required pharmacy licenses and permits.
  • Develops and implements policies and procedures for medication management and reconciliation processes for home infusion associates to include pharmacist consultation criteria and methods of identifying at-risk patients and procedures for performing medication regimen interventions with patients and physicians.
  • Maintains effective working relationships with other departments and participates in multi-disciplinary quality and service improvement teams. Participates in meetings and on committees. Collaborates effectively with nursing intake billing and other departments to ensure seamless patient care coordination. Communicates regularly with referral sources and managed care organizations. Serves as a clinical resource for internal associates patients and external healthcare providers.
  • Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations. Encourages growth and development of direct reports.
  • Provides strategic and operational leadership for the Home Infusion Pharmacy department. Develops implements and monitors departmental goals objectives and budgets. Manages pharmacy personnel including recruitment hiring orientation training competency assessment performance evaluations and disciplinary actions. Fosters a culture of safety quality and continuous improvement. Oversees the maintenance of all required associate pharmacy licenses and certifications.
  • Participates in and facilitates participation in-home medication services education and marketing efforts with internal and external customers as part of business development plans. Prepares and participates in company presentations in-services etc. for internal and external customers and regulatory agencies as applicable.
  • Is responsible for medication quality program oversight to include error and intervention tracking clinical documentation and observational auditing of medication reconciliation process and development of preventive measures and processes aimed at improvement of home health medication management. Develops implements and monitors the pharmacy's Performance Improvement (PI) program collecting and analyzing data identifying opportunities for improvement and implementing corrective actions. Coordinates with the compounding pharmacy to ensure the quality and sterility of all compounded sterile preparations (CSPs) through robust quality control processes including environmental monitoring personnel qualification and media fill testing. Oversees the medication management process including procurement storage compounding labeling dispensing and disposal.
  • Participates as part of the management team of MedStar Health Homecare in developing new business opportunities and formulating and maintaining strategic business plans in order to achieve clinical and financial outcomes. Evaluate internal and external market for business opportunities to maximize appropriate utilization of pharmacy services. Performs ongoing monitoring and evaluation of programs to assure their value to the organization
  • Manages inventory purchasing and supply chain to ensure availability of necessary medications and supplies. Provides strategic oversight of medication procurement while ensuring strict adherence to 340B program requirements and compliance guidelines.
  • Oversees the maintenance and calibration of all pharmacy equipment ie infusion pumps. Maintains an inventory and tracking of infusion pumps and obtains additional pumps as needed. Verifies the training and competency of pharmacy associates with pump programming and verification

Minimal Qualifications
Education

  • Bachelor's degree in Pharmacy required 
  • Master's degree in HealthCare Administration MBA or Pharmacy preferred 
  • Doctoral degree in Pharmacy - PharmD preferred
  • One year of relevant education may be substituted for one year of required work experience.

Experience

  • 5-7 years of progressive experience in home infusion pharmacy with 3-5 years in a leadership or management role. required
  • One year of relevant professional-level work experience may be substituted for one year of required education.

Licenses and Certifications

  • RPH - Registered Pharmacist - Pharmacist license in the State of Maryland required and apply for Pharmacist licensure in WDC and VA within 90 Days required 
  • Board Certification in Sterile Compounding (BCSCP) and other relevant certifications within 90 Days required

Knowledge Skills and Abilities

  • Excellent interpersonal analytical writing and presentation skills
  • Basic managerial finance skills and ability to perform financial (cost/benefit) analyses
  • Comprehensive understanding of home care pharmacy operations with the ability to apply this knowledge in analyzing operations
  • Excellent written and verbal communication skills. Basic computer skills preferred

USD $161,158.00 - USD $348,296.00 /Yr.
