General Summary of Position
Responsible for initiating and maintaining ongoing professional contacts with non-MedStar hospitals and physicians MedStar Outpatient Clinics and physician practices skilled nursing facilities and health facilities to develop skilled and infusion home care referrals. Responsible for covering at hospitals and MHHC branch offices as may be warranted to provide referral assistance. Additionally is responsible for developing relationships with health organizations medical and physician societies health agencies and other appropriate healthcare groups to identify their home care needs and to educate them on MHHC services. Contributes to the coordinating of referral for home care in the excess of 35000 referrals annually.
Primary Duties and Responsibilities
- Contributes to the achievement of established goals and objectives and adheres to department policies procedures quality standards and safety standards. Completes annual Code of Conduct Compliance and other mandatory training. Complies with governmental and accreditation regulations.
- Coordinates and communicates between referral source and home care agency to obtain physician home care orders and treatment plans to facilitate scheduling home care services.
- Develops and coordinates home care referrals by developing relationships and working on-site or off-site on an ongoing basis with the case managers social workers physicians in the skilled nursing facilities hospitals assisted living facilities clinics and physicians' offices to identify and meet their patient's home care needs. Acts as consultant to referral source to assess the appropriateness of their patients for home care; and resource on home care issues guidelines and practices.
- Expand the referral network by establishing and maintaining a professional working relationship with existing hospitals skilled nursing facility and physicians; looks for opportunities for development of new business; build awareness of services and home care guidelines by planning and conducting presentations and educational seminars.
- Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.

- Records and maintains referral visits and activity; submits weekly reports reflecting customer contacts referrals concerns outcomes and market and/or competitor activity.
- Reviews and accepts physician orders as necessary on behalf of the community account managers.
- Coordinates care and develops home care referrals by monitoring electronic referral systems and specialty programs.
Minimal Qualifications
Education
- Bachelor's degree preferred
Experience
- 5-7 years 4 years licensed practitioner preferred; experience in home care setting strongly preferred preferred and
Licenses and Certifications
- Valid LPN PTA OTA or Social Work license in the District of Columbia Maryland or Virginia or any combination as required based on work location(s) Upon Hire required and
- DL NUMBER - Driver License Valid and in State (DRLIC) with access to transportation Upon Hire required and
- Must be insured for his/her state's minimum personal injury and property damage liability Upon Hire required
Knowledge Skills and Abilities
- Ability to prepare and present multiple written and graphic reports to groups with a high degree of accuracy.
- Self-directed team player with strong interpersonal skills including verbal and written communication.
- Basic computer skills.

USD $61,838.00 - USD $111,259.00 /Yr.
