General Summary of Position
Collaborates with providers to ensure safe and effective use of specialty medications and improves communication among the patient's healthcare team by reviewing and evaluating clinical guidelines formulating protocols conducting medication reconciliation creating patient care plans and providing patient consultation that will improve patient medication adherence health well-being and knowledge. Documents and reports on healthcare-related interventions and outcomes. Participates in the process of maintaining specialty pharmacy accreditation.
Primary Duties and Responsibilities
Education
- Doctoral degree PharmD from an accredited college of Pharmacy required
- 3-4 years 3+ years of direct patient care experience in a clinical role preferably in a specialty pharmacy required and
- Ambulatory care experience preferred
- PHARMD - Licensed Pharmacist in Maryland or the District of Columbia as applicable to practice site Upon Hire required and
- Eligibility for reciprocity to surrounding states as required for practice Upon Hire required and
- Completion of an ASHP accredited residency or other approved advanced training program Upon Hire preferred or
- Board of Pharmacy Specialty certification (BPS) or other approved certification like Certified Specialty Pharmacist (CSP) AAHIVP Upon Hire preferred
- Verbal and written communication skills.
- Basic computer skills preferred.
This position has a hiring range of
USD $120,702.00 - USD $238,222.00 /Yr.
