What You'll Do Deliver Exceptional Clinical Pharmacy Services- Review patient records and medication therapies for appropriateness, safety, and effectiveness.
- Evaluate medication orders and prescriptions for clinical and operational concerns.
- Develop and recommend individualized medication therapy plans that optimize outcomes and minimize adverse effects.
- Assess patient conditions to ensure all medication-related needs are addressed.
- Monitor patient progress and therapeutic outcomes through chart reviews, patient profiles, and interdisciplinary collaboration.
- Identify, prevent, and resolve medication-related problems and drug interactions.
- Provide recommendations regarding drug selection, dosing, administration, contraindications, and therapy adjustments.
- Practice Medication Therapy Services through patient assessment and evidence-based pharmaceutical care recommendations.
- Partner closely with physicians, advanced practice providers, nurses, and other healthcare professionals to ensure optimal patient care.
- Serve as a clinical resource for medication-related questions and consultations.
- Routinely advise providers on medication availability, formulary considerations, side effects, incompatibilities, and care pathway management.
- Participate in quality improvement initiatives, drug utilization evaluations, and adverse drug reaction reporting.
- Serve as a drug information resource for medical, nursing, and pharmacy staff.
- Present educational programs, in-services, and lectures to support ongoing staff development.
- Precept and mentor pharmacy students and pharmacy residents.
- Assist in developing staff competencies related to patient care and pharmacy practice.
- Participate in departmental initiatives, policy reviews, and medication-use evaluations.
- Contribute to investigational protocols and research activities as needed.
- Complete required pharmacy inspections, expiration-date checks, and floor stock reviews.
- Maintain compliance with regulatory, licensing, accreditation, and organizational standards.
- Bachelor of Science (BS) in Pharmacy or Doctor of Pharmacy (PharmD) from an accredited school of pharmacy.
- Current Pharmacist license in the state of practice.
- Strong clinical judgment, communication skills, and commitment to patient-centered care.
- Ability to work effectively in a fast-paced healthcare environment and manage competing priorities.
- Commitment to maintaining patient confidentiality and adhering to safety standards.
- Two or more years of clinical pharmacy experience.
- Completion of a PGY-1 Pharmacy Residency.
- Missouri pharmacists: Medication Therapy Services Certificate issued by the Missouri Board of Pharmacy within 90 days of hire.
Successful candidates will demonstrate proficiency with:
- Epic and other pharmacy information systems for order verification and clinical documentation.
- Drug information resources such as Micromedex.
- Pharmacy automation technologies including Omnicell, McKesson Automation, ScriptPro, Baxa Systems, and related platforms.
- Strong clinical and analytical skills.
- Excellent interpersonal and communication abilities.
