Find your calling at Mercy! The Pharmacy Resident II is a participant in a Post-Graduate year 2 (PGY2) pharmacy residency program that builds on Doctor of Pharmacy (Pharm.D.) education and Post-Graduate year 1 (PGY1) pharmacy residency programs to contribute to the development of clinical pharmacists in specialized areas of practice. PGY2 residencies provide residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. PGY2 competency areas, goals, and objectives in advanced areas of pharmacy practice are differentiated from those from PGY1 by specialization and the expectation of PGY2 residents for greater work competence and proficiency in the specialty area.

The PGY2 pharmacy resident works with members of the health care team to provide safe and effective drug therapy and clinical patient care services. Serves as a drug information resource to pharmacy, nursing, and medical staffs as well as to patients. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards. Position Details: Clinical Pharmacist – PGY1 Residency Required Position Summary

The Clinical Pharmacist is responsible for ensuring safe, effective, and evidence-based medication therapy management while collaborating with interdisciplinary teams to optimize patient outcomes. This role involves direct patient care, education, quality improvement, and participation in departmental and organizational initiatives.

Key Responsibilities Patient Care & Clinical Practice
  • Review patient medical records to ensure appropriateness of medication therapy.
  • Evaluate patient conditions and identify untreated or undertreated issues.
  • Develop and implement individualized medication plans that minimize risk of adverse effects.
  • Assess outcomes of pharmaceutical treatments and adjust therapy as needed.
  • Monitor drug therapy and disease state management through chart reviews and patient interactions.
  • Provide recommendations on dosing, drug selection, administration, contraindications, and drug interactions.
  • Collaborate with physicians, nurses, and other healthcare professionals to optimize patient care.
  • Maintain accurate, timely documentation of medication plans and patient progress.
Medication Safety & Risk Management
  • Adhere to confidentiality policies and procedures.
  • Utilize institutional reporting systems to document patient safety concerns, including medication-related events and near misses.
  • Evaluate medication orders for clinical appropriateness and operational accuracy.
  • Participate in adverse drug reaction reporting and drug utilization evaluations.
  • Support compliance with regulatory and accreditation standards (e.g., Joint Commission).
Care Coordination & Collaboration
  • Coordinate efforts with pharmacy team members and departmental staff.
  • Partner with providers and staff across departments to ensure continuity of care.
  • Routinely consult with physicians and nurses regarding medication appropriateness, availability, side effects, and formulary considerations.
  • Maintain strong professional relationships across disciplines.
Education & Professional Development
  • Serve as a preceptor for PGY1 residents and Pharm.D. students.
  • Provide in-services, lectures, and training to healthcare staff.
  • Assist in developing and validating staff competencies related to patient care practices.
  • Maintain licensure and complete continuing education requirements.
  • Stay current with evolving clinical practices and institutional policies.
Quality Improvement & Research
  • Participate in quality assurance initiatives, including identifying issues and recommending solutions.
  • Contribute to continuous process improvement, pilot programs, and business plan development.
  • Engage in investigational protocols and research projects as applicable.
  • Support departmental initiatives such as Medication Use Evaluations (MUE) and policy reviews.
  • Participate in ministry-wide committees (e.g., Pharmacy & Therapeutics, specialty councils).
Operational & Technical Competency
  • Demonstrate proficiency with health information systems, including order entry and verification.
  • Prioritize daily responsibilities effectively while demonstrating sound clinical judgment and fiscal responsibility.
Qualifications Required
  • Education: Bachelor of Science in Pharmacy (BSPharm) or Doctor of Pharmacy (PharmD); PharmD preferred
  • Residency: Completion of PGY1 Pharmacy Residency
  • Licensure: Eligible for pharmacist licensure; must obtain state licensure within 120 days of hire
  • Missouri Requirement: Medication Therapy Services certification by start date
Preferred
  • Resident Teaching Program Certificate
  • Experience in clinical pharmacy practice or specialized care areas
  • Active involvement in research, presentations, or clinical committees
Additional Expectations
  • Completion of all required competencies and continuing education
  • Fulfillment of residency program requirements per organizational policy
  • Active participation in departmental and organizational initiatives
