- Identifies real and perceived barriers to establishing or maintaining the patients plan of care and develops plans with specific goals and interventions to address these barriers. The care planning process is interdisciplinary and considers bio-psychosocial functions.
- Provides care management for patients with complex medical conditions, demonstrating exceptional assessment and planning skills. Deploys supportive resources using appropriate intervention levels to meet the patients needs, without fragmenting or duplicating the care.
- Facilitates timely, safe, and appropriate transition of patients from one care level to the next, such as transition from/to home, emergency department, inpatient, or other post-acute care settings.
- Ability to work in home or field-based settings, as well as effectively utilize telephonic, virtual visits, or other technology in management of the individual.
- Works closely with the inpatient care management teams for effective patient handoffs.
- Excellent communication & collaboration skills with interdisciplinary care teams (PCP, cardiology, hospitalists, ED, NOC, home health, hospice, social worker, dietary, rehab, infusion center, post-acute facilities).
- Capable of engaging the patient and family to assist in identification of their values and goals as they relate to their medical condition, functional status, and end-of-life decisions. Assures care and treatment are oriented toward achieving and maintaining each patients functional independence and providing compensatory support where necessary.
- Addresses individual gaps in care with patients and/or caregivers as indicated.
- Demonstrates knowledge of community resources and when/how to access the resources.
- Demonstrates knowledge of the legal and ethical issues related to confidentiality when working in case management, and with various health plans.
- Works independently, within the scope of their license with minimal direction and in collaboration with the care team to develop and implement provider-directed plan of care.
- Documents case management activities timely and according to departmental policies and other nationally established standards.
- Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards. Position Details:

Education: Graduation from registered nursing program. Bachelors (or more advanced) Degree in Nursing.
Licensure: Current and unrestricted license as a Registered Professional Nurse in the State(s) of which the nurse will practice.
Certification/Registration: CCM or RN-BC professional case management certification preferred or attained within 36 months of hire.
Other skills & knowledge: Excellent interpersonal skills. Above average human verbal and written skills. Ability to communicate effectively at many different levels including patients, providers, health system leaders, families, other health professionals. Knowledge and basic proficiency at keyboarding, Microsoft Word, Microsoft Outlook, Microsoft Excel, Microsoft PowerPoint. Understanding of managed care, utilization, quality improvement concepts and principles. Ability to work in a stressful environment, prioritizing and managing multiple tasks, and function within an ever-changing environment. Capable of completing a head-to-toe assessment, use of critical thinking skills and problem-solving. Has experience with pathophysiology and heart/lung assessment of patients with cardiovascular and respiratory conditions.
Working Conditions, Mental and Physical Requirements: Fast moving environment, requiring flexibility and adaptability in multiple environments: primarily telephonic visits with other type of visits possible such as the clinic or hospital.
Equipment Used: Routine office equipment, laptop or desktop computer, webcam, copy machines, fax machines, telephones, cell phones.
