Job Description:
The Palliative Medicine Service Community Based Nurse Practitioner reports to the Palliative Medicine Service Manager and works collaboratively with the Interdisciplinary Palliative Team as well as patient's larger health care team. Caseload consists of community based palliative patients and their families. The Palliative Nurse Practitioner provides specialty palliative care and management of the patient with serious and/or chronic illness, in collaboration with the supervising physician and through implementation of standardized procedures. The NP possesses skills and knowledge to provide excellent patient care, offers a high level of compassion and comfort discussing end-of-life issues and pain and symptom management to develop the patient's plan of care. The NP will be responsible for coordinating community based palliative services, including provider and patient education. The NP will manage their caseload to include patient level coordination of care.
Experience
Requirements:
Two or more years of experience in working with patients and families on an interdisciplinary team. Excellent assessment, diagnosis and treatment skills. Effective communication and public relation skills. Self-directed, motivated and possess strong organizational skills.
Preferences:
Two or more years of experience in palliative care, oncology, and/or hospice. Knowledge and experience with advanced health care directives and POLST. Knowledge and experience with bereavement care, working with grieving individuals and families.
Education
Master's Degree in Nursing required- Exception Nurse Practitioners' qualified or certified as an NP in California or any other state prior to January 1, 2008
Licensure/Certifications
Must be currently licensed in California as an NP and be certified as a nurse practitioner by a recognized national certifying body that has established standards for nurse practitioners to practice in the State of California. Certification from either ANCC or AANP as a nurse practitioner. Current RN licensure and a registered professional nurse with authorization in California to practice as a nurse practitioner in accordance with California law and be certified as a nurse practitioner by a recognized national certifying body that has established standards for nurse practitioners. A furnishing license issued by the Board of Registered Nursing and a current DEA number from the United States Drug Enforcement Administration for schedule II-IV controlled substances is required. American Heart Association Healthcare Provider BLS is required. Palliative Care certification preferred.
Must possess a valid California driver's license.
This position will require driving. In addition to a valid CA driver's license, the following are required: Automobile insurance as required by the State of California with an acceptable driving record of three points or less, participation in the DMV pull-notice program, and an automobile available for daily work.
