Job Summary

Based in the Prisma Health Richland Level 1 Adult and Level 2 Pediatric trauma center, a newly established hospital-based violence intervention program (HVIP) aims to prevent and reduce community and interpersonal violence among high-risk youth and adults. This role will manage and facilitate all aspects of the HVIP including project planning, implementation, coordination, and tracking. This role is responsible for the oversight and coordination of intensive case management and wrap around services provided by the HVIP. This role is also responsible for training, communications, quality assurance, data management, program reporting and resource development, including grant writing for the HVIP and related services.

Accountabilities

*Coordinate and facilitate all aspects of the HVIP including project planning, implementation, coordination, and tracking and execute the project according to timelines, budget and within the required level of quality.

*Day-to-day monitoring including documentation & case reviews with two Violence Prevention Professionals and one Behavioral Health Counselor; monitor staff caseload sizes and levels of service needed.

*Collaborate and maintain relationships with hospital and community violence intervention and prevention partners and stakeholders; attend and participate in collaborative meetings regarding service coordination.

*Research, develop new, and maintain current relationships with community service providers to secure additional resources for program participants.

*Coordinate training activities for team member case managers; meet regularly with team members to provide individual support and guidance regarding case plan implementation; accompany team members on field work as needed to provide training, assistance, give guidance/feedback and to assess performance.

*Oversee program budget and expenses.

*Support and oversee consistent and accurate documentation of services in electronic databases and paper files.

*Develop written materials and communication such as annual reports, briefing documents, abstracts, agendas, news articles, and other publications; represent Prisma Health's HVIP to the media, public officials, community leaders, etc. on violence prevention strategies. 

*Lead efforts to obtain new funding such as researching funding opportunities and writing grant proposals to secure funding and ensure program sustainability; prepare program and budget reports to funders as required.

Supervisory/Management Responsibilities

This is a non-management job that will report to a supervisor, manager, director or executive.

Minimum Requirements

Bachelor's Degree in public health, social work, psychology, or related field.

Master's degree preferred.

3 years of experience with case conferencing and/or working with high-risk youth and adults.

Must have reliable transportation, valid driver's license, and a certificate of auto insurance.

Knowledge, Skills or Abilities

Candidates with personal knowledge and experience in overcoming violence/ violence-related injuries/ the criminal justice system preferred.

Knowledge of local community issues, specifically community and youth violence preferred.
