What You Will Do:

As an RN Case Manager, you will coordinate and oversee the interdisciplinary plan of care for patients receiving skilled home health services. You will collaborate with patients, families, physicians, social workers, therapists, caregivers and community partners to ensure care is safe, timely and aligned with each patient's goals.

In this role, you will:

  • Assess patients in their homes and develop individualized plans of care.
  • Provide skilled nursing services, education and hands-on clinical care.
  • Coordinate care with physicians, therapists, social workers and other members of the interdisciplinary team.
  • Monitor patient progress and adjust the care plan as needs change.
  • Educate patients and caregivers about medications, treatments, symptom management and safety.
  • Help patients successfully transition from hospitals, rehabilitation facilities and other care settings back into their homes.
  • Connect patients and families with appropriate community resources and support services.
  • Maintain accurate, timely clinical documentation using Epic.
  • Promote patient safety, quality improvement and cost-effective care.
  • Manage a daily patient schedule with a high level of independence and flexibility.

What You Will Need

  • Associate degree in Nursing required; bachelor's degree in Nursing preferred.
  • Current Wisconsin RN license or RN license through an eligible Enhanced Nurse Licensure Compact state.
  • Valid Wisconsin driver's license and an acceptable driving record.
  • Current BLS certification or ability to obtain certification upon hire.
  • Previous clinical nursing experience preferred.
  • Home health, hospice, case management, pre-acute or post-acute care experience preferred.
  • Experience using Epic or another electronic health record system preferred.
  • Strong assessment, communication, organization and patient education skills.
  • Ability to work independently, prioritize changing patient needs and coordinate care across multiple disciplines.
