Certification Details
• AHA certifications only required for all MS/TELE units
• Current Health Form/Physical Fitness for Duty signed and dated by provider within a year of start
• Fit Mask - must be updated annually
• PPD or IGRA test within 12 months of start
• Positive Mumps Titer or 2 MMR vaccines
• Positive Rubella Titer or 1 MMR/Rubella Vaccination
• Hepatitis B requirements specific to area
• Varicella documentation required
• Drug Testing - specific lab instructions provided
• Vaccinations - Seasonal Flu and COVID-19 preferred
• Eye Exam - Snellen required
• Criminal Background Check - must be within last 3 months
