Title: Care Transition Manager

Regular Company: Roswell Park Comprehensive Cancer Center Department: Nursing - Inpatient Administration Time Type: Full time Weekly Hours: 40 FTE: 1 Shift: First Shift (United States of America) Summary: Manages the daily operations of in-home care services provided to patients. Ensures that quality, compassionateness, and compliant care is delivered in accordance with state regulations, agency policies, and patient needs. Manages Care Transition Associates, coordinates scheduling, monitors service delivery, and serves as a key liaison between clients, families, caregivers, and agency leadership.Starting annual salary is $86,681 which includes a comprehensive benefits package. Qualifications: Required Education and Experience
1. Master's degree in a business or health related field and the equivalent of two (2) years of full-time experience in home care, healthcare management, or related field; or
2. Bachelor's degree in a business or health related field and the equivalent of four (4) years of full-time experience in home care, healthcare management, or related field; or
3. Associate's Degree in a business or health related field and the equivalent of six (6) years of full-time experience in home care, healthcare management, or related field; or
4. High School Diploma or High School Equivalency Diploma and the equivalent of eight (8) years of full-time experience in home care, healthcare management, or related field.
NOTE:
Required degrees must have been granted by an accredited school, college or university or one recognized by Roswell Park Comprehensive Cancer Center as following acceptable educational practices.
Preferred Qualifications
The ideal candidate should have a minimum 10 years of clinical experience in a homecare setting, demonstrated managerial experience in healthcare, and proven success in launching and developing healthcare programs.
