***Paygrade effective 7/1/2025 $64.55-$103.20 per hour***

Summary of Position: 

 The MDS Coordinator & Nurse Supervisor - SNF promotes and restores residents' health by developing day-to-day management and long-term planning of the resident care area; directing and developing staff; collaborating with physicians and multidisciplinary professional staffs; and providing physical and psychological support for residents, friends, and families.  This position also conducts and coordinates the development and completion of resident assessment in accordance with current standards and regulations to include resident interview, assessment, evaluation and care planning. They are responsible for the education of all clinical and nonclinical staff in the Skilled Nursing Facility as well as Case Management duties in the SNF. 

Additionally, this position addresses resident care, staffing as needed, and any other issues that may arise. The MDS Coordinator & Nurse Supervisor – SNF addresses staff and resident incidents during the shift, ensuring appropriate documentation and communication. They determine the issues to be referred to the Director of Nursing. 

 

Qualifications

Education:

• Completion of an accredited Nursing program.

• Bachelor's degree in Nursing (BSN) preferred.

• Completion of Director of Staff Development (DSD) training program approved by California Department of Public Health (CDPH)

 

Experience:

• Minimum two years recent experience in designated department or similar setting.

• Minimum two years recent supervisory experience preferred.

• Long Term Care and Skilled Nursing Facility experienced required

• MDS experience required

 

Knowledge/Skills/Abilities:

• Previous experience with MDS Assessments required

• Knowledge of SNF consolidated billing, Medicare Skilled criteria, Medicare eligibility requirements

• Knowledge of nursing practices in designated department. 

● Highly effective written and verbal communications skills. 

● Proficient computer skills as are required to document, communicate and enter information into the electronic medical records system.

● Requires critical thinking skills, decisive judgment, and ability to work with minimal supervision.

● Must be able to work in a stressful environment and take appropriate action.

● Ability to collaborate with various departments in and outside of the Barton Health System in order to provide consistently exceptional patient care. 

• Attention to detail, meticulous development and maintenance of regulatory paperwork

 

Certifications/Licensure:

• Current California RN license

• Current CPR/BLS certification by the American Heart Association (BLS for the Healthcare Provider)

• MDS Certification, or ability to obtain within 6 months of hire

Essential Functions

1. Provides consistently exceptional care at all times.

2. Schedules and coordinates and completes the development and completion of resident assessment (MDS) in accordance with current standards and regulations to include resident interview, assessment, evaluation and care planning. Ensures timely submission of MDS' to CMS:

a. Works with interdisciplinary Care Plan team in the developing a comprehensive care plan for each resident and ensures that the plan

b. Meets the resident's medical, nursing and psychosocial needs

c. Is in accordance with resident wishes

d. Outlines care to be provided

e. Goals to be accomplished to include measurable objectives and timeframe goals

3. Contact person for interdisciplinary care plan team and coordinates resident care planning conferences. Leads skilled resident rounds daily with therapy and nursing. 

4. Coordinates and maintains policies and procedures that guide the development and implementation of the resident assessment (MDS) and care plan.

5. Collects data, completes reports and assists in maintaining an on-going QAPI program

6. Conducts performance Improvement Projects as recognized by the facility, reports to QAPI and documents in Point Click Care Insights. 

7. Coordinates and develops MDS staff training activities to include assessment and care planning skills and techniques and importance of ensuring corporate compliance

8. Demonstrates the knowledge and skill necessary to identify and meet the individualized needs of all residents served in Skilled Nursing.

9. Responsible for all education of clinical and non-clinical staff in the Skilled Nursing Facility. These responsibilities include, but are not limited to:

a. Develops curriculum and conducts job specific orientation coordinating classroom and floor orientation activities for all licensed and CNA Staff.

b. Coordinates annual skills labs for licensed and unlicensed staff.  Coordinates as monitors competencies for all licensed staff

c. Coordinates and manages RN new Grad Program in the SNF for new RN hires

d. Attends Education Council meetings.

e. Ongoing education is provided secondary to the identified needs of the staff. Educational needs are identified through 

i. On-going evaluation and monitoring activities (all shifts) through

ii. Feedback from staff, residents and physician

iii. New technologies and standards. 

f. Develops curriculum, goals, objectives and lesson plans for educational offerings specific to skilled nursing and long-term care.

g. Maintains files and necessary documentation to ensure compliance with regulatory and licensing standards.

h. Completes medication administration competency verifications for all licensed staff annually. 

i. Ensures that all staff have on file current licenses/certifications, documented initial competencies (job specific orientation) and on-going education and competencies.  Maintaining records per regulations

j. When opportunities for improvement are identified, works with Director of Nursing and employee to create a development plan to meet the needs of the employee and unit. Conducts Just Culture remedial training for clinical staff as needed

10. Completes psychotropic medication monitoring monthly for anti-psychotics, and anti-anxiety medications as evidenced by completion of psychotropic monitoring form placement in chart, and discussion with IDTeam regarding possible reduction in medications for residents in Long Term Care per state and federal regulation

11. Completes all prior authorizations for medications not covered by Medicare

12. Covers for Resident Relations Coordinator in her absence, assisting with family communications and MDS section and care plan completion. 

13. Is responsible for Case Management of short-term skilled rehabilitation and Long Term Care admissions, transfers and discharges. This includes:

a. Communication with entities wishing admission of resident

b. Assessment of resident to ensure appropriateness of admission

c. Coordination with family and/or Case Management of transferring facility 

d. Coordination with facility staff for admission date, etc

e. Communicates with accepting MD

f. completes PASRR for all admissions and follows up with any Level II findings

g. Receipt of admission orders, consents as appropriate.

h. Communication with entities if resident not appropriate for admission to SNF

i. Assists Social Services with maintaining and updating the waiting list in Point Click Care.

14. As a member of the Leadership team in the SNF, assists with supervisory duties such as:

a. Coordinating staffing in the facility to ensure adequate staff ratios

b. Assists in Just Culture investigations and counselling of staff

c. “On the spot” training and coaching of staff 

d. Performs the duties of the Director of Nursing in his/her absence

15. Completes all CNA performance reviews

16. Assists with staffing and scheduling for nurses and CNAs.

17. Assists with annual employee goal monitoring, completion and documentation

18. Conducts and documents monthly RNA meetings with RNA, PT, OT

19. Participates in new department projects and assists in completion. 

20. Steps in to cover as charge nurse, medication nurse as needed. 

21. Takes administrative call after hours and weekends. 

22. Services are provided based on the ages of the population served; Adult through geriatric in collaboration with the multidisciplinary care team.

23. Responds to the needs of the department by performing other duties, as necessary.
