Position Responsibilities Summary: The Nurse assists residents with routine daily nursing care and services in accordance with their assessment and care plan in order to meet their physical, emotional and psycho-social needs. The Nurse possesses the experience and knowledge to work in both Assisted Living and Memory Care communities. Directly Supervises: Memory Care and Assisted Living Resident Assistants Indirectly Supervises: Memory Care and Assisted Living Resident Assistants Key Leadership, Management and Accountabilities (LMA): The Nurse provides assistance with medical, clinical, personal care, household, recreational, social, dining and transport/transfer/mobility needs; takes and records vital signs as directed; assists in the administration of medications under the direction and supervision of the Director of Health and Wellness; observes and documents the health, condition and eating habits of each resident, promptly reporting any changes to the nurse; promotes personal safety and a safe environment for residents; encourages residents to be as independent as able in daily living activities; uses and maintains equipment and supplies appropriately; ensures proper inventory of individual resident care items and supplies; compassionately assists with end of life care, etc. Additionally, the Nurse supervises the Memory Care and Assisted Living Resident Assistants. NURSING and CLINICAL ASSISTANCE * Assist residents with routine daily nursing care and services in accordance with their assessment and care plan. * Provide assistance with residents' medical, clinical, personal care, transport/transfer/mobility, household, dining, recreational and social needs. * Assist in the administration of medications. * Take and record vital signs and chart pertinent information in the resident's medical record, as directed. * Observe and document the health, condition and eating habits of each resident, promptly reporting any changes to the Director of Health and Wellness. * Promptly report injuries, safety issues and changes in residents' condition and/or needs. * Assist in the admission and discharge processes. * Compassionately assist with end of life care. RESIDENT SUPPORT * Act as residents' advocate. * Build relationships with and provide care and support to residents and their families. TRAINING, SAFETY AND COMPLIANCE * Supervise and train Caregivers/Resident Assistants. * Promote personal safety and a safe environment for residents. * Encourage residents to be as independent as they are able in their daily living activities. * Understand safety procedures and respond appropriately. * Use proper infection control and standard precaution procedures. * Stay up to date on all emergency policies and procedures and participate and assist in drills within community. * Assist community care team in maintaining Residential Care Facility and OSHA compliance. Education/Certification Requirements: * High school diploma. * Associate's or Bachelor's Degree in Nursing preferred * Valid Nursing License in the State of Ohio: LPN, LPV, or RN Skills/Qualifications Requirements: * One (1)+ year experience in a similar role, preferably from within a Long Term Care, Assisted Living and/or Memory Care community. * Prior supervisory experience preferred * Strong customer service, interpersonal and communication skills (English). * Keen attention to detail. * Caring and compassionate disposition. * Ability to perform physical tasks such as lifting, moving and transporting residents. * Flexibility: the Nurse will be required to work flexible hours to meet the needs of the community. Work hours shall remain flexible to meet the needs of the community, its residents, staff and operations. Position may require working weekend and evening hours and/or on-call status. * PC literacy. * Valid driver's license and auto insurance. Travel Requirements: * Limited travel as directed, for various staff and client care service needs, and to attend required training and conferences. Work Environment: * Office Environment: high-paced office/branch environment; continuous phone use/ringing; talking; various noise levels; interactions with employees and clients; sitting and standing for long periods of time; bending and stooping required; lifting up to 10 lbs. regularly and up to 50+ lbs. may be required; using a computer for extended periods: typing, staring at a bright monitor or TV screen; using fingers, wrists, and arms; stress on back may occur; possible exposure to prescription drugs, bodily fluids, small appliances for cooking, cooling, and heating foods or securely storing medications. * Resident's Environment: Unknown environments; bodily fluids, household and human germs; extreme cold or hot temperatures in or outside; light to heavy traffic; inclement weather; human body weights that vary based on client health and lifting as prescribed or needed per plan of care; 50+ lbs.
