A Day in the Life of a Social Worker
As a Social Worker at our rural healthcare facility, you will perform comprehensive assessments of patients' physical, psychological, and social needs to craft targeted care plans. Your role involves coordinating mental health services, substance abuse programs, and long-term care solutions, ensuring tailored interventions for each patient. You will face challenges such as managing complex cases with sensitivity and maintaining high standards of patient confidentiality. Collaboration with multidisciplinary teams and community agencies is crucial to optimize care and connect patients with necessary resources. The work structure emphasizes both independent decision-making and teamwork, supported by detailed documentation and communication processes.

In Hospital Care Management at CRMC, we ensure that patients receive the right care, at the right time, in the right setting—even after they leave our hospital. From coordinating discharge plans and arranging follow-up care to addressing social determinants of health, our team ensures a smooth transition from hospital to home, rehab, or other care facilities. We are patient advocates, problem solvers, and resource navigators, ensuring that no patient falls through the cracks. Qualifications
Education and Experience Required
- Current Minnesota Social Workers license
- If not licensed, must obtain within 90 days of hire
- Graduate of an accredited program of Social Work or a Bachelors degree in Social Work from a nationally or regionally accredited college or university
- MSW
- Three years experience as a social worker in the acute care setting
The pay range listed above is the estimated starting pay range for this role. Actual pay rate offered depends on many factors including each individual employee's experience level, preferred education/certifications, along with market conditions.
