We want everyone who works at CVS Health to feel empowered by the role they play in transforming our culture and accelerating our ability to innovate and deliver solutions to make health care more personal, convenient and affordable. *Position Summary* Care Management Associate will help to support the Individual and Family Plan (IFP) Membership - Responsible for initial case review including confirming eligibility using appropriate systems available prior to initiating outreach - Completes assignment of members to appropriate Case Managers based on defined assignment criteria i.e. availability and licensure - Will receive inbound calls from members and must accurately and consistently documents each call in the members electronic record, thoroughly completing required actions with a high level of detail to ensure Compliance and Regulatory Standards are met - Ability to be agile, managing multiple priorities and adapting to change with enthusiasm - Determined to build strong relationships with peers and our Aetna members - Demonstrates an outgoing, enthusiastic, and caring presence over the telephone *Required Qualifications* - Experience with computers including knowledge of Microsoft Word, Outlook, and Excel - 2 years of experience preferably in customer service, telemarketing and/or sales - Ability to work 8-5pm, M-F - Strong organizational skills, including effective verbal and written communications skills *Preferred Qualifications* - Data entry and documentation within member records is preferred - Background in health care setting a plus - Call center experience preferred - Familiarity with basic medical terminology preferred *Education* - High school diploma, GED, or equivalent experience *Pay Range* The typical pay range for this role is: $18.50 - $35.29This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.    In addition to your compensation, enjoy the rewards of an organization that puts our heart into caring for our colleagues and our communities.  The Company offers a full range of medical, dental, and vision benefits.  Eligible employees may enroll in the Company's 401(k) retirement savings plan, and an Employee Stock Purchase Plan is also available for eligible employees.  The Company provides a fully-paid term life insurance plan to eligible employees, and short-term and long term disability benefits.
