We want everyone who works at CVS Health to feel empowered by the role they play in transforming our culture and accelerating our ability to innovate and deliver solutions to make health care more personal, convenient and affordable. *Position Summary* This is a full-time field-based teleworker opportunity in Oklahoma.  Qualified candidates preferably will reside within the region which includes the following counties: Creek, Okfuskee, Seminole, Pontotoc, Okmulgee, Hughes, Coal, Atoka, Bryan, Wagoner, Muskogee, McIntosh, Pittsburg, Atoka, Bryan, Choctaw, Pushmataha, Latimer, Haskell, Cherokee, Adair, Sequoyah, Le Flore, Pushmataha, Choctaw, and McCurtain. Community Health Workers (CHW) serve the local community as a bridge between the member and family and the healthcare/social services system through member contact, face-to-face coaching sessions, outreach and education. By providing these services, CHW's help Aetna Better Health members attain and maintain better health outcomes, have access to and fully utilize needed social supports, improve relationships with health care practitioners/providers, and help all parties learn about each other's culture. The CHW's role is meant to assist with access to services and to improve the quality and cultural competence of service delivery. Fundamental Components: * Engage members who are experiencing, or are at risk of, health disparities, to improve their engagement, access to care, and their overall health. * Conduct home visits with members to reinforce following the care plan, adjusting tasks to fit with the member's culture, language, and religion, improve access to care, and identify health care and social needs. * Obtain and educate members about available SoonerSelect Program services. * Educate healthcare professionals and providers about protective health factors and the social determinants of health that may be impacting member's health and treatment plan through various communication channels. * Advocate on behalf of the member and their preferences with respect to receiving member and family-centered care. * Build and maintain positive working relationships with the members, their circle of support, providers, care coordinators, and other health plan support staff.   * When applicable, serve as a member of the Interdisciplinary Care Team (ICT) and the  multidisciplinary case rounds supporting the member to bridge information between the member and the care team regarding any identified cultural and/or psychosocial elements. * Work to reduce cultural and socio-economic barriers between members and institutions. * Assist with care plan implementation and member education during in-home visits, help develop care management strategies, and work with team members to provide linkages that address the various health and social needs of members. * Assist members to identify socio-economic issues that affect their overall health and develop  health/social management plans and goals. * Identify gaps and opportunities to strengthen systems of care within the member's community and assist members in utilizing community services, including scheduling appointments with social services agencies, (including transportation vendors) and assisting with completion of applications for eligible programs. * Assist members to access community-based resources to address non-medical needs and to support the member's Care Plan objectives and independence. * Support medication adherence by reviewing the member's current medication list and assist in obtaining refills. * Accompany members to appointments as needed or requested. * Provide support to member with filing grievances and appeals. * Outreach, engage, and support members for re-enrollment and primary care provider (PCP) or provider assignment. * Enter and maintain member records in electronic health record system, compile reports and complete other program documentation in a timely manner. * Travel extensively to client homes, community locations, various agencies, and other outreach destinations. * Ability to effectively provide support care to socially and medically complex patients in a variety of non-traditional settings. * Exceptional organizational and interpersonal skills, with attention to detail required. * Ability to work collaboratively in a team and manage multiple priorities, utilizes effective time management skills, and exercise sound judgment. * Ability to influence medical care providers as needed to adjust interventions based on member's cultural, ethnic, or religious preferences. * Strong interpersonal and communication skills and the ability to work effectively with a wide range of constituencies in a diverse community. * Knowledge of regional formal and informal community agencies and resources. * Working knowledge of multi-system outreach programs related to health care delivery, clinical education, and health-related services. * Ability to communicate medical information to health care professionals and care coordinators over the telephone. * Requires flexibility to work in situations in which the working conditions are unknown, as environments and situation of patient homes may vary. *Required Qualifications* * Must reside in Oklahoma in specified areas * If not certified, completion of a Community Health Worker CHW/Promotora approved certification program recognized by the State of Oklahoma is required within 12 months of start date. * 3-5 years' relevant experience working in a community-based position or in a mental health capacity * Meet all requirements set forth by the state (i.e. first aid, CPR, TB screen, etc). * Experience with Electronic Health Records. * 2+ years of demonstrated proficiency with personal computers, keyboard, multiple systems navigation, and MS Office Suite applications (Outlook, Word, Excel, SharePoint). * Requires the ability to travel to multiple office locations and home locations. * Must possess reliable transportation and be willing and able to travel in-state up to 50-75% of the time. Mileage is reimbursed per our company expense reimbursement policy. *Preferred Qualifications* * 1-2 years' experience providing community services; experience in community/outpatient setting preferred. * Community Health Worker Certification is preferred at time of hire. * Experience in serving the Medicaid Populations in urban or rural environments with familiarity of local formal and informal resource networks preferred. * Preferred skills: Mental Health First Aid, Trauma Informed Care, HIPPA, C.L.A.S. Standards, Motivational Interviewing, and specialized training in medical terminology and practices for designated health conditions. * Experience providing care to American Indian/Indigenous American/Native American populations *Education* * High School Diploma or G.E.D., or equivalent experience *Pay Range* The typical pay range for this role is: $18.50 - $31.72This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls.  The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.  This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.    In addition to your compensation, enjoy the rewards of an organization that puts our heart into caring for our colleagues and our communities.  The Company offers a full range of medical, dental, and vision benefits.  Eligible employees may enroll in the Company's 401(k) retirement savings plan, and an Employee Stock Purchase Plan is also available for eligible employees.  The Company provides a fully-paid term life insurance plan to eligible employees, and short-term and long term disability benefits.
