The Licensed Practical Nurse is responsible for administering medications, conducting nursing assessments, maintaining accurate records, monitoring medication inventories, and communicating effectively with clients and staff. Key Responsibilities:
Administer medications to clients via various methods, ensuring proper dosage and adherence to protocols.
Conduct comprehensive nursing assessments, including physical, mental health, and psychiatric evaluations.
Maintain accurate and timely documentation of medication administration, assessments, and client interactions.
Monitor medication inventories, ensuring adequate stock and timely refills for residential clients.
Communicate effectively with clients, addressing medication queries and coordinating refill requests with providers.
Verify client data, medical history, and visit purposes, facilitating seamless care delivery.
Participate in nursing-led educational groups and contribute to client safety planning.
Assist with general lab duties and phlebotomy as needed.
Serve as a resource for nursing and medical issues, promoting client hygiene and infection control measures.
Actively participate in required meetings and perform other assigned duties as delegated by nursing leadership.
Education and/or Experience Qualifications:
Current licensure as a Licensed Practical Nurse (LPN) in the state of practice.
Graduation from an accredited practical nurse program.
Required License/Certification (one of the following):
Licensed Practical Nurse (LPN)
Additional Qualifications:
Successful completion of background check including criminal record, driving record, and abuse/neglect.
Valid driver's license in state of residency with an acceptable driving record.
Strong communication skills, both verbal and written, with the ability to produce clear and concise reports.
Proficient in accurately entering, transcribing, recording, and storing information in various formats.
Skilled at reading and interpreting various documents, including laboratory results and electronic health records.
Competent in basic mathematical calculations.
Possesses strong problem-solving abilities and adaptability.
Demonstrates a willingness to learn, grow, and work collaboratively with diverse perspectives.
