Job Title: RN Care Manager
Job Description
As an RN Care Manager, you will be responsible for clinical assessment, member-centered care plan development, ongoing care coordination, quality assurance, and monitoring of services to program members who may be older adults and/or individuals with intellectual or physical disabilities. Your role will involve achieving member outcomes and goals through adherence to standards of care and best practices. You will interact with members and their families with respect, compassion and integrity, and participate in continuous quality improvement efforts. You will also contribute to enhancing work experiences, participate in training and development opportunities, and support operational goals. You will conduct home visits and assessments, use critical thinking to optimize members' health and quality of life, develop and maintain care plans, and educate members and their families.

Job Type
This is a Contract-to-Hire position with a duration of 6 Month(s).
Work Site
This is a hybrid position requiring some days on-site in Mequon, Wisconsin and some days remote.
Work Environment
You will manage your schedule based on members' needs, primarily during day shift hours. You may be required to visit members at home or in long-term care facilities. While you can work from home, you may also have access to an office for training and internal meetings. Initial training will mostly occur in the office for the first 3-6 months. After you have familiarized yourself with the role and built trust, you will have the flexibility to work remotely on some days. However, we generally ask staff to be in the office at least 3 days per week.
