We have multiple openings for Registered Nurses to support the Department of Veterans Affairs Medical Center, Eastern Colorado Healthcare Service. These long-term contract opportunities will involve on-site work performed in Aurora, CO and/or Glendale, CO depending upon needs (40 hours/week, Monday – Friday, between 7:30 AM – 5:30 PM).
TASKS/SERVICES. The contractor shall:
(1) Provide patient-centered, evidence-based care by contributing to the patient's assessment, determining priorities, identifying nursing measures and therapeutic objectives and evaluating outcomes.
(2) Clinically review consults/requests for completeness, appropriateness, and utilization review
(3) Actively participates in care coordination, liaising with inpatient and outpatient units to facilitate continuity of care, foster interdisciplinary approach to treatment, and respond to medical emergencies as directed.
(4) Provide clinical review of incoming medical records documenting communication with ordering providers and primary care physician
(5) Responsible for coordination of care, which involves corresponding with veteran, community providers, internal providers/nurses, and administrative staff to ensure the care and authorization for care requested is processed.
(6) Triage specialty consults for clinical appropriateness and completeness within the National Referral Coordination Initiative (RCI) Standards for coordinated care.
(7) Communicate with veterans on their health care options via phone calls to determine receiving care at VA facility or through the community eligible providers so the veterans can make informed decisions on how/when/where to receive specialty care if they are determined to be eligible
(8) Serves as an advocate for patients, families, and caregivers, improving outcomes for the immediate practice setting.
(9) Provide clinical knowledge to the veterans and act as a clinical resource to multidisciplinary staff both within and outside the Veterans Integrated Service Networks (VISN)
(10) Serve as a liaison to both internal staff and community care staff during the consultation process.
(11) Serve as Delegation of Authority by building trusting relationships with the specialty providers in the clinics based on specialty triage tools with the VA specialty providers
REQUIREMENTS. The contractor must have:
(1) Completion of nursing degree from program approved by the appropriate state-accrediting agency and accredited by the Commission for Education in Nursing (ACEN) or The commission on Collegiate Nursing Education (CCNE).
(2) Current license in at least one (1) state, territory, or commonwealth in the U.S. or the District of Columbia.
(3) Current Basic Life Support certification
(4) Minimum of two (2) years of direct patient care experience within the last three (3) years.
(5) Specialized nursing experience in areas such as infusion, hematology, surgery, PACU, behavioral health, rehabilitation, and/or home health preferred
(6) Ability to complete process for credentialing and privileges, as applicable, prior to providing services
(7) Ability to utilize independent judgement along with an interdisciplinary approach in the provision of nursing care.
(8) US Citizenship and ability to obtain a government-sponsored security clearance required as per contract requirements
