General Summary of Position
Initiates and maintains ongoing professional contacts with MedStar Outpatient Clinics and physician practices non-MedStar hospitals physicians skilled nursing facilities and health facilities to develop skilled and infusion home care referrals. Develops relationships with health organizations medical and physician societies health agencies and other appropriate health care groups to identify their home care needs and to educate them on home care services. Contributes to the coordinating of referrals for home care in the excess of 35000 referrals annually.
Primary Duties and Responsibilities
- Contributes to the achievement of established department goals and objectives and adheres to department policies procedures quality standards and safety standards. Complies with governmental and accreditation regulations.
- Coordinates and communicates between referral source and home care agency to obtain physician home care orders and treatment plans to facilitate scheduling home care services.
- Coordinates and develops home care referrals independently by developing relationships and working with on an ongoing basis the case managers/social workers in the hospitals skilled nursing facilities clinics assisted living facilities urgent care centers and physicians (physician offices) to identify and meet their patients' home care needs.
- Evaluates referral source satisfaction by monitoring outcomes of service and referral volume while pursing input to program planning evaluation and timely feedback. Reviews satisfaction scores and creates action plan to address any deficiency.
- Expands the referral network by establishing and maintaining a professional working relationship with existing hospital physicians (physician offices) clinics assisted living facilities urgent care centers and skilled nursing facility customers; builds customer awareness of agency services by planning and conducting presentations and educational seminars.
- Maintains and records customer visits and referral activity.
- Maintains effective working relationships with other departments and participates in multidisciplinary quality and service improvement teams. Participates in meetings and on committees and represents the department and agency in community outreach efforts. Enhances growth and development and enriches personal knowledge and skill through participation in educational programs and affiliations.

Minimal Qualifications
Education
- Bachelor's degree required or
- an equivalent combination of education and experience. required
Experience
- 1-2 years Related experience in a healthcare setting required
Knowledge Skills and Abilities
- Ability to prepare and present multiple written and graphic reports to groups with a high degree of accuracy.
- Basic computer skills.
- Self-directed team player.
- Strong interpersonal skills including verbal and written communication.
- Ability to communicate with patients their relatives physicians and a variety of personnel
This position has a hiring range of
USD $65,062.00 - USD $117,291.00 /Yr.
