General Summary of Position
Provides front line leadership to the Community Health Advocate team at assigned facility. Performs the responsibilities of a Community Health Advocate.


Primary Duties and Responsibilities

  • Contributes to the achievement of established department goals and objectives and adheres to department policies, procedures, quality standards, and safety standards. Complies with governmental and accreditation regulations.
  • Advocates for individual and community health equity.
  • Assists patients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and /or social services.
  • Assists the Program Coordinator in monitoring performance issues. Contributes to the performance evaluation process by providing feedback to the Program Coordinator/Manager and assisting with the creation of professional development plans for Community Health Advocate staff members.
  • Collects, tracks and reports information about the community and community benefits.
  • Documents all patient encounters; completes and submits monthly reports; maintains comprehensive electronic client files, which include client notes, release of information, assessments and other medical documents acquired on behalf of the patient. Documents activities, service plans, and outcomes achieved by patient in an effective manner.
  • Enhances community residents' ability to effectively communicate with health care providers.
  • Identifies departmental performance improvement opportunities and demonstrates a solution-oriented approach in addressing those issues.
  • Integrates with patients care team to support progress in care plan and overall patient wellness.
  • Manages the team in the Program Coordinator's absence. Keeps Program Coordinator informed about issues related to staffing and problem areas. Keeps Program Coordinator informed about issues related to quality, risk, patient/family issues and concerns, and allocation of resources.
  • Monitors and arranges for adequate staffing to ensure appropriate Community Health Advocate coverage for hospital. Monitors daily workflow issues and addresses issues related to same in collaboration with Program Coordination, as necessary.
  • Oversees the orientation of new Community Health Advocate hires by establishing the plan and monitoring progress in conjunction with other staff, as necessary.
  • Performs all responsibilities of a Community Health Advocate.
  • Participates in meetings and on committees and represents the department and hospital in community outreach efforts. Participates in multi-disciplinary quality and service improvement teams.

    Minimum Qualifications
    Education

    • High School Diploma or GED required or
    • equivalent. required and
    • Residency within the target population preferred

    Experience

    • 1-2 years 2 years experience as a community health advocate required and
    • Participation with a community organization, verified by personal or professional reference with personal knowledge of this fact preferred

    Licenses and Certifications

    • Cert Community Health Worker - CCHWMD Certified Community Health Worker in State of Maryland required

    Knowledge, Skills, and Abilities

    • Effective oral and written communication skills.
    • Personal knowledge of the target population, as shown by residency in that neighborhood.
    • Cultural competency. Knowledge of local resources and system navigation.
    • Advocacy and community capacity building skills.
    • C are coordination skills.
    • Teaching skills to promote healthy behavior change.
    • Outreach methods and strategies.
    • Ability to bridge needs and identify resources. Understanding of ethics and confidentiality issues.
    • Ability to use and understand health information technology.
