General Summary of Position

MedStar Health is looking for a Treatment Navigator to join our team at MedStar Health Research Institute!  

The Treatment Navigator helps identify, link, retain and provide overall coordination of care in medical care, medical case management, support services, health education programs, and research opportunities. Coordinates the collaborative efforts of treatment navigation for patients across a network of city-wide providers. Engages hard to reach patients to achieve ultimate health outcomes.

Primary Duties: 

  • Consistently demonstrates support for compliance in the performance of job duties and responsibilities by a) developing and maintaining knowledge of, and performing job functions in compliance with, rules, regulations, policies, and statutes that affect his/her job.  

  • Maintains knowledge of and performing job functions in compliance with MedStar Health and MedStar Health Research Institute policies and procedures. Reports compliance concerns appropriately and timely.  

  • Maintains clear, accurate and up-to-date documentation in the EMR (Electronic Medical Record). Participates in case conferencing with the care team to share pertinent information about the patient's current environment and medical status.  

  • Participates in case finding activities. Participates in needs assessment and networking activities. Participates in testing and counseling efforts in the Emergency Department and other locations as assigned.  

  • Referral tracking and follow-up services. Works closely with care team to identify unmet needs that might impact patient wellness and assist the team with prioritizing those needs. 

 

Qualifications:  

  • High School Diploma or GED. 

  • 1-2 years' experience completing needs assessments. Familiarity with patient resources in the local surrounding area. 

  • Mental Health First Aid within 6 months of hire. 

  • Some college preferred. 


This position has a hiring range of $45,843 - $79,892
