Overview:

 

The Care Management model provides effective transition planning and length of stay oversight while ensuring quality patient care and safety. The seamless care model supports patient throughput, healthcare team collaboration, and coordination of care through optimized performance and adoption of best practices.
The Social Service Associate Care Manager, as part of the Care Management team, performs duties and responsibilities in a manner consistent with the Mercy mission, values, and Service Standards.


Qualifications:

 

Required Education

Associate Degree in Psychology, Sociology, Science, or related field

 

Preferred Education

Bachelor's Degree in Psychology, Sociology, Science, or related field

 

Required Experience

2 years or greater acute care hospital setting

 

Required Certification/Registration

BLS (CPR) at hire date or within 90 days
