NorthBay Health's Integrative Primary Care (IPC) Program offers a highly personalized primary and preventive care experience featuring outstanding internal medicine, family practice, lifestyle and integrative medicine providers. This is not a traditional primary care practice. Our IPC program physicians adopt a personalized, relationship based philosophy in a highly consultative setting. They have a smaller than standard primary care participant panel and work in close collaboration with the on site lifestyle and integrative medicine team – RN's, dieticians, movement, stress management, sleep specialists as well as medical acupuncturists, to provide patient participants a seamless, high touch, highly personalized Integrative Primary Care patient experience. The program is dedicated to providing outstanding primary care services and coordinated access to our network of NorthBay specialists. The Integrative Primary Care Program is not a concierge medicine practice however all program patient participants do pay a program fee, not covered by insurance. 

At NorthBay Health, the Integrative Primary Care RN in close collaboration with the Program MD, will provide complex care management in highly personalized setting. The RN is responsible for delivering an individualized primary care and preventive care experience focused on enhancing each patient's healthcare experience. The position fosters a collaborative relationship between patients, their provider and on site Integrative Primary Care team members to ensure the delivery of an enhanced Integrated Primary Care, concierge-like patient experience. The RN position serves as the direct liaison between provider and patient to ensure that patient's questions and concerns have been addressed. This position also plays an active role in program development participating in patient education, participation and outreach goals, conducting new patient member orientation, managing projects and promoting awareness. The ideal candidate personally embraces Lifestyle medicine fundamentals and has a desire for continuous growth and development in the professional practice of nursing.   

PRIMARY JOB DUTIES

The RN clinical duties include but are not limited to: 

  • Collaborate as part of a 1:1 MD/RN care team within the Integrative Primary Care Program.
  • Manage complex care for an outpatient population with chronic health conditions.
  • Demonstrate a high level of service orientation in an outpatient program where patients pay a fee for access and participation.
  • Independently carry out clinical RN duties in an outpatient setting without the support of a medical assistant, phlebotomist, or administrative staff including:
    • Comprehensive patient assessment
    • Take vitals, maintains exam rooms 
    • Phlebotomy-ordering/fulling/preparing personalized lab specimens 
    • Counsel patients
    • Coordinate lifestyle and integrative medicine activities
    • Incorporate lifestyle education into the interdisciplinary plan of care
    • Coordinates referrals and follow up
    • Schedules testing
    • Fulfills EHR documentation requirements 
    • Documents in other portals as program components require
    • Maintaining patient correspondence and communication in a timely and efficient manner
  • Adheres to policies, industry standards, best practices 
  • Collaboratively participates in program development activities such as conducting new patient member orientation, managing projects and promoting awareness.
  • Promotes a quality, high touch, high service patient experience at all times


Qualifications

Education: 

  1. Graduated from an accredited nursing school or college. 

    1. BSN is required (excludes those employees grandfathered).

 

Licensure/Certification: 

  1. Current California state RN license required. 

    1. AHA or equivalent BLS required.

 

Experience: 

  1. Minimum two (2) years RN experience. 

    1. Phlebotomy experience required, or ability to complete training prior to opening of program operations.  

    2. Case management experience preferred.

 

Knowledge of: CMS, TJC and EMTALA standards and federal and state regulations. Knowledge of telephone-based clinical assessment techniques. Current NCQA and HEDIS quality standards, trends in health care, best practices, management tools, and familiarity with related community resources and literature. 

Skills: 

  1. This position will ensure that the patient experience provided at all levels exceeds the expectations of physicians, patients and team members and is provided in a financially responsible manner. 

  2. Computing skills including experience with MS Office (Excel, Word, PowerPoint, etc.) and data using electronic medical records to check patient history and documentation.  management.

  3. Ability to effectively communicate with all levels of patients, physicians, health care personnel, supervisory staff, and peers and elicit information needed to make brief evaluations. Effective public speaking skills for patient education and program development required.

  4. Demonstrates the exceptional ability to be organized and efficient in prioritizing and managing assignments with minimal oversight and direction. 

  5. Demonstrates the willingness to research, learn, and obtain knowledge for the performance of the position. 

  6. Demonstrates a courteous, professional demeanor at all times either working individually or in a team environment working collaboratively in an effective manner maintaining positive relationships with peers and supervisors alike. 

  7. Utilizes critical thinking and applies sound clinical judgment and assessment skills for decision making. 

  8. Must be able to function in a multi-task environment.

Hours of Work: Based on business needs.

 

Compensation: $70.69 to $85.93 based on years of experience doing the duties of the role.
