Summary

Job Summary:
The pharmacist ensures the safe and effective use of medications by providing medication order verification while assessing for optimal drug product selection, dosage regimen design, effective administration, and drug therapy monitoring services.  Monitors impatient and outpatient drug therapies when appropriate and works with relating medical staff to find alternatives when needed, to ensure the best care possible is being given.

Responsibilities:
 

• Review and assess all medication orders to identify, prevent, and resolve potential and actual drug-related problems
• Communicate with medical staff as necessary to facilitate appropriate patient care
• Compound and dispense medications accurately for patient use, including sterile, chemotherapy and parenteral nutrition products
• Complete pharmacokinetic consultations for medication dosing
• Monitor drug therapy care plans for safety and efficacy
• Provides clinical consultation and clarification to practitioners; suggests appropriate, cost-effective therapeutic alternatives to medical staff.
• Provide medication education to patients/family as needed

Qualifications:

Education & Experience:  Must have a Pharm.D. 

• Licensed by the State of Kansas Board of Pharmacy required OR must be obtained within 90 days of hire.
• Basic Life Support (BLS) and Advanced Cardiovascular LIfe Support (ACLS) required OR must be obtained within 90 days of hire.
