Home Care Scheduler - Onsite Pewaukee - .9 FTE

Schedule Details: This .9 FTE commits to 36 hours per week, 4 full days and 1 half day per week. Holiday rotation required, though only work half day each holiday. This position is required to work onsite at the Riverwood Admin building. This is not a seasonal or temporary position.

Job Summary The Home Health Scheduler is responsible for coordinating all activities related to scheduling staff and patient assignments for ProHealth Home Care and Hospice.  Specifically, accurate scheduling, registering and entering data of patient information into the Epic system. Responsibilities Enters employee and patient schedules accurately into the computer-scheduling program to generate employee and patient schedules. Promptly handles and documents changes due to patient or employee cancellations with appropriate notification of patient and/or staff.  Provides agency personnel with current demographic information related to assignments.   Schedules staff for orientation to special skilled cases in a consistent manner as directed by coordinator.   Gives timely input to coordinator/manager and other staff to coordinate staffing issues such as admissions, high volume and problem-prone cases. Advises coordinator/manager in a timely manner of staffing needs such as overtime assignment and replacement staff.   Consults with the coordinator/manager regarding problems related to scheduling and/or personnel or when joint visits are scheduled.   Communicates and documents all cancellations, schedule changes and employee status changes to patients and to appropriate agency personnel.  Completes updating of all scheduling records prior to shift change and weekends with copies provided to designated personnel.   Addresses patient concerns promptly and courteously. Brings to the attention of the coordinator and/or manager, potential problems or special concerns regarding patients and personnel.   Receives and records all messages from patients, and reports from staff appropriately, and forwards to R.N. case manager and other appropriate staff.   Assists with telephone contacts for the agency as requested, relaying messages and/or transferring calls to appropriate agency personnel in a courteous and prompt manner; determines prioritization of incoming messages and acts accordingly.    Assigns requested HCA.   Participates in aspects of the orientation of new staff, including scheduling of observation visits. 
Coordinates staff schedules with PTO, holiday and weekend rotation. Education High School Diploma - or equivalent What You Will Do:

What you will need:

  • High school diploma or equivalent.
  • At least 1 year of related scheduling and/or health care experience preferred.
  • Experience with Epic EMR preferred.
