- Special requirements: assists with durable medical equipment, home health and multiple types of post-acute placements such as Skilled Nursing Facilities, adult foster homes, and assisted living. Works with multiple community partners to arrange plans and secure authorization from insurance companies. This role does not provide utilization review, but knowledge of UR is helpful.

- Ability to float between units as required.
- Availability to work occasional weekends as needed.
- Capacity to handle patient ratios of 1:17.
- Skills in assisting with durable medical equipment, home health, and arranging post-acute placements.
