We are seeking a highly skilled and compassionate Social Worker/Case Manager to join our Health Equity and Access team. This position offers an exciting opportunity to work on a dynamic team dedicated to improving healthcare access and ensuring equitable care for individuals and families with chronic health conditions. As the Social Worker/Case Manager, you will be responsible for providing direct services to clients and their families, as well as assisting with developing and implementing an appropriate plan of care to meet their specific needs. This includes offering education, support, and access to all available social services.

Qualifications:

Qualifications:

  • Bachelor's degree in psychology, social work, counseling, or a related field.
  • Valid AR driver's license required with current automobile insurance and reliable car for moderate travel to LR occasionally and to surrounding communities (mileage reimbursed at the current state mileage reimbursement rate) if a state vehicle is not available—occasional out-of-state travel.
  • A personal working cell phone is required.
  • Must be able to work evenings and weekends as needed.

Knowledge, Skills & Abilities:

  • Certification or licensure as a Social Worker (e.g., LCSW, LMSW).
  • Proven experience in case management or social work, preferably in a healthcare setting.
  • Knowledge of healthcare systems, social services, and resources available in the community.
  • Excellent communication and interpersonal skills, with a demonstrated ability to build rapport with diverse populations.
  • Strong organizational and time management skills, with the ability to prioritize and manage multiple tasks.
  • Ability to work independently and as part of a team.
  • Experience with Microsoft Office is required.
  • Excellent oral and written communication skills.
  • Ability to work under deadline pressure.

Preferred Qualifications:

  • Master's degree in Social Work or related field preferred.
  • Familiarity with electronic health record (EHR) systems is preferred.


Additional Information:

Responsibilities:

  • Promotes Screenings, Consultation, and Oversight of Case Management Procedures and Processes: Conducts initial screenings and assessments to identify client needs and eligibility for healthcare services. Collaborates with the Assistant Director and other team members to develop and implement effective case management processes and procedures. Ensures compliance with established guidelines and protocols.
  • Develop and Manages Relationships with Program Participants and Community Stakeholders: Builds and maintains strong relationships with program participants, their families, and community stakeholders. Coordinates case conferences and collaborates with other healthcare and social service professionals to ensure comprehensive care. Advocates on behalf of clients and their families to ensure access to resources, services, and benefits.
  • Performs Quality Checks for Case Management Plans Delivery: Conducts regular quality checks to ensure that case management plans are being effectively implemented and meeting client needs. Identifies areas for improvement and makes recommendations for modifications to existing plans. Provides feedback and guidance to team members regarding case management practices.
